What Is The Failure Analysis Policy?

A customer may request Failure Analysis, a Failure Analysis Request Form (attached) is required to be completed by the Customer and returned to QLogic. QLogic Quality representative will review the FA request and validate the customer information warrants FA. Once Quality approval is received Customer Care will provide an FA number to the customer for product return.


In Warranty:

  • QLogic will provide a replacement board for product undergoing FA due to the potentially damaging analysis performed on the actual board returned for FA
  • Once FA is complete, QLogic will return the tested board with an FA report to the engineering contact (indicated on the FA form) for test correlation.

Out of Warranty:

  • QLogic will not perform Failure Analysis on out of warranty product.


End of Life:

  • No FA will be performed on product that has been announced End of Life by QLogic regardless of warranty status unless contractually agreed to by both parties.

SWITCHES:

In Warranty:

  • QLogic will perform FA on switches as requested and return the switch to the customer upon completion of the FA testing with an FA report.

Out of Warranty:

  • Out of Warranty charges will apply if the switch is OOW.


End of Life:

  • No FA will be performed on product that has been announced End of Life by QLogic regardless of warranty status unless contractually agreed to by both parties.
