VMware KB2149780, below, has been resolved as of June 2017.
The current solution is to update to ANY "out of box" QLogic driver. All newer drivers resolve this issue.Details from the Original VMware KB:
Details from the Cavium Application Note: VMware ESCi 6.0 U3 Driver
- Upgrading to ESXi 6.0 U3 causes the Qlogic FC HBA's not to be detected
- Upgrading the Qlogic driver from older version to 2.1.50 causes the FC HBAs to disappear from ESXi
New HBA device IDs are not available in inbox driver
This is a known issue with VMware ESXi 6.0.x.
Currently, there is no resolution.
To workaround this issue, uninstall the 2.1.50 version of Qlogic driver and install 2.1.44 async driver or latest async driver.
Note : If you are affected by this issue - ESXi host or virtual machine hangs after approximately 85 days. For more information see ESXi host or virtual machine hangs after approximately 85 days (2147739), then install the driver version 2.1.44.

inbox qlnativefc version 188.8.131.52 driver does not discover all supported Host
The VMware ESXi 6.0 U3 inbox qlnativefc 184.108.40.206 driver is missing device
IDs for the HPE and QLogic Host Bus Adapters listed in the Products Affected
table. These adapters do not appear in the storage adapters list; in addition, the
esxcli storage core adapter list does not show these Host Bus Adapters.
Install a qlnativefc out-of-box driver.
VMware KB article #2149780 on this subject is not entirely correct. Any version of
the out-of-box drivers provided directly from HPE or VMware does not have this
