HBA from "offline" to "running". SCSI midlayer may change a device's
state to "offline" if it could not get response from the device. I/O's
with such devices are ignored by SCSI midlayer.
For example if a SCSI command timeouts on a particular device and fails
to recover the device. The SCSI midlayer will mark the device as
"offline". Later when the device is online or accessible the state needs
to be changed to running or online. The ql-lun-state.sh utility changes
