Return Material Authorization (RMA) number(s) can be obtained by completing the
Online RMA Request Form below. This form is for customers or partners providing
service on behalf of end-customers who have purchased QLogic branded products
from an authorized partner.
Model Numbers, Part Numbers, and Serial Numbers are located on product Bar Code Labels on the product. Some data may be available from the management tools. All fields in BOLD must be filled out in order to submit the form.
